Healthy Illinois 2021
The State Health Improvement Plan (SHIP), State Health Assessment (SHA), and State Innovation Model (SIM) are three statewide initiatives that focus on improving the health of Illinois residents through strategic approaches and engaging all aspects of the public health system. Collectively, these are called Healthy Illinois 2021.
