The State Health System Innovation Plan

At the end of the SIM work the State will have a State Health System Innovation Plan. This plan will identify ways to improve population health across the State. It will also include a plan for making changes to the health care delivery system, which may include changes to the workforce, health information technology, and payment systems. One of the major goals for the plan is that it is specific and actionable and can be implemented with available resources.
