Nursing Home Violations For The Month Of January

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of January. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Biltmore Rehab & Nursing Center, an 80-bed skilled and intermediate care facility located at 1701 W. 5th Avenue, Belvidere, has been fined $10,000 for failure to identify residents who are at risk of being abused and neglected, to ensure all allegations of abuse/neglect are reported to the administrator and thoroughly investigated, and to initiate a plan to prevent further abuse of residents. This neglect began when a resident verbally and physically abused other residents. The facility has not requested a hearing on the Department’s action.

Clearbrook-Wright Home, a 16-bed intermediate care facility for the developmentally disabled located at 34377 N. Almond Rd, Gurnee, has been fined $5,000 for failure to implement protocols to prohibit a resident from leaving the facility without staff knowledge. The facility requested a hearing on the Department’s action. A status in the matter is set for July 17.

Countryview Care Center-Macomb, a 62-bed skilled and intermediate care facility located at 400 W. Grant St., Macomb, has been fined for failure to notify the physician about a change in a resident’s condition. The facility was also fined for failing to provide and/or seek immediate attention for the resident. The resident was admitted to the hospital for emergency care. The facility requested a hearing on the Department’s action and paid a $13,000 fine.

Four Seasons Living Center, a 38-bed skilled care facility located at 303 N. Jackson St., Morrison, has been fined for failure to provide for the safety of a resident, by not assuring the stairwell alarm reset procedure was known to all staff. The facility also failed to develop protocols to prevent a resident from wandering and failed to have fall prevention interventions in place. A resident in a wheelchair attempted to leave the facility without staff knowledge and fell down the steps of an exit hallway. The facility requested a hearing on the Department’s action and paid an $11,500 fine.

Garden Center Services, a 15-bed intermediate care facility for the developmentally disabled located at 8345 S. Austin Ave., Burbank, has been fined for failure to implement its policy to prohibit neglect after a resident sustained second degree burns while in the bath tub. The facility requested a hearing on the Department’s action, in protest of the $20,000 fine.

Hallmark House Nursing Center, a 71-bed skilled care facility located at 2501 Allentown Rd., Pekin, has been fined $20,000 for failure to administer insulin as ordered and for failing to notify a resident’s physician when the resident’s blood sugar level dramatically dropped. The resident was given ten times more insulin than the physician ordered. The resident required emergency hospitalization. The facility requested a hearing on the Department’s action. A status in the matter is set for August 2.

Helia Healthcare of Urbana, a 99-bed skilled care facility located at 907 N. Lincoln Ave., Urbana, has been fined $20,000 for failure to ensure that a resident had the proper medical supplies, while out on a pass, to monitor blood sugar levels and administer insulin, and failed to educate the resident and the family about the possible need for insulin. These failures resulted in the resident being transported to the hospital for emergency care. The facility has not requested a hearing on the Department’s action.

Kanthak House, a 16-bed intermediate care facility for the developmentally disabled located at 724 Second Ave., Ottawa, has been fined $10,000 for failure to prevent abuse by implementing its policies and procedures that prohibit mistreatment, neglect or abuse of residents. The facility did not investigate allegations of abuse, nor did they take appropriate action when they became aware of an arrest for a disqualifying offense by a staff person and allowed the staffer to continue to work with residents of the facility. The facility has requested a hearing on the Department’s action.

Manorcare at Rolling Meadows, a 155-bed skilled care facility located at 4225 Kirchoff Rd., Rolling Meadows, has been fined $20,000 for failure to implement policies and procedures to protect a resident from being sexually assaulted by another resident with a history of inappropriate sexual behavior. The facility also failed to notify local law enforcement of the incident. The facility has requested a hearing on the Department’s action. A status in the matter is set for September 17.

Marklund @ Mill Creek 4, a 16-bed intermediate care facility for the developmentally disabled located at 1 S. 410 Wyatt Dr., Geneva, has been fined $20,000 for failure to implement its policy to prohibit neglect when they failed to ensure residents were properly transferred and repositioned. These failures resulted in residents sustaining injuries after slipping/falling. The facility has requested a hearing on the Department’s action. A status in the matter is set for July 25.

Morris Healthcare & Rehab Center, 142-bed skilled and intermediate care facility located at 1338 Clay St., Morris, has been fined $50,000 after an incident where a resident fell from bed and broke a hip. The resident was hospitalized and later died. The facility has requested a hearing on the Department’s action.

Rehab & Care Center – Jackson County, a 202-bed skilled care facility located at 1441 N. 14th St., Murphysboro, has been fined $15,000 for failure to prevent a staff member from deliberately agitating a resident. The staffer, who used a cell phone to videotape a resident while on the commode, was fired. The facility has requested a hearing on the Department’s action. A status in the matter is set for August 15.

Renaissance at Hillside, a 178-bed skilled care facility located at 4600 N. Frontage Rd., Hillside, has been fined $25,000 for failure to ensure that a resident was free from physical abuse by a staff person. The staffer had a previous finding of abuse entered into the state nurse aide registry. Failure to check background resulted in this person working at the facility and injuring a resident, who required hospital care. The facility has requested a hearing on the Department’s action. A status in the matter is set for August 14.

River Bluff Nursing Home, a 304-bed skilled care facility located at 4401 N. Main St., Rockford, has been fined $20,000 for failure to develop and initiate a monitoring policy regarding Resident on Resident Abuse. The facility also neglected to conduct a thorough investigation of a resident’s sexually inappropriate behavior toward four other residents. The facility requested a hearing on the Department’s action. A status in the matter is set for August 27.

River Court, a 4-bed intermediate care facility for the developmentally disabled located at 760 E. River St., Kankakee, has been fined $11,000 for failure to implement policies and procedures to protect residents from mistreatment, neglect or abuse. The facility did not monitor trends of a resident’s falls and injuries, conduct re-assessments as needed, and implement interventions to protect the resident from further injury. The facility also failed to notify the guardian promptly of condition changes and incidents. The facility requested a hearing on the Department’s action. A status in the matter is set for August 27.

Riviera Manor, a 200-bed skilled and intermediate care facility located at 490 W. 16th Pl., Chicago Heights,has been fined $5,000 for failure to properly supervise and monitor residents who have a known and documented unsafe behavior of smoking in their rooms. Residents were found smoking at bedside, with noted cigarette burn holes on bed sheets and chairs. Records show no intervention on these unsafe smoking practices. The facility requested a hearing on the Department’s action. A status in the matter is set for August 20.

Seguin RCA Harvey House, a 16-bed intermediate care facility for the developmentally disabled located at 3309 S. Harvey Ave., Berwyn, has been fined $20,000 for failure to implement policies to prevent neglect for a resident with a history of falls. The resident was admitted to the hospital with a skull fracture that was probably caused by a fall. The facility has requested a hearing on the Department’s action. A status in the matter is set for July 23.

Springwood Nursing & Rehab, a 97-bed skilled and intermediate care facility located at 1920 N. Main St., Rockford, has been fined $40,000 for failure to assess and identify risks to residents and implement a care plan to prevent injury. This resulted in three residents sustaining serious injuries. The facility has requested a hearing on the Department’s action. A prehearing is set for August 17.

Tammerlane Health Care Centre, a 70-bed intermediate care facility located at 3601 Sixteenth Ave., Sterling, has been fined $20,000 for failure to prevent a resident from being physically, verbally and mentally abused by a staff person. The staffer did not follow the facility policy to defuse an over reactive behavior incident with the resident. The staff person attempted to physically lift the resident and twisted the resident’s hand. The resident was treated at a local hospital for a sprained shoulder and cut hand. The facility has requested a hearing on the Department’s action. A status in the matter is set for August 6.

The Wealshire, a 144-bed skilled, intermediate and sheltered care facility located at 150 Jamestown Lane, Lincolnshire, has been fined $10,000 for failure to initiate cardiopulmonary resuscitation (CPR) on a resident with Advanced Directives. The resident was pronounced dead upon arrival at the hospital. The facility has requested a hearing the Department’s action. A status in the matter is set for August 7.
