NURSING HOME VIOLATIONS FOR THE MONTH OF JANUARY

SPRINGFIELD – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of January. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Knox County Nursing Home, a 204-bed skilled care facility located at 800 N. Market St., Knoxville, has been fined $5,000 for failure to inspect slings used to move residents, such as from a bed to a chair; to revise the policies and procedures for checking slings; to train staff on how to check a sling; and to remove a defective sling that ripped apart when a resident was being moved. The resident fell to the floor and suffered a back bruise. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Pinnacle Health Care of Berwyn, L.L.C., a 145-bed skilled care facility located at 3601 S. Harlem Ave., Berwyn, has been fined $50,000 for failure to follow a physician’s order requiring a pureed diet for a resident with a known chewing and swallowing problem. The resident was given a peanut butter sandwich, choked on the food and suffered a stroke. The resident died at a local hospital following the incident. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Countryside Healthcare Center, a 197-bed skilled and intermediate care facility located at 1635 E. 154th St., Dolton, has been fined $15,000 for allowing a social worker to continue to work the rest of his shift after residents reported to staff that he had been abusive to another resident. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Chicago Ridge Nursing Center, a 231-bed skilled care facility located at 10602 Southwest Highway, Chicago Ridge, has been fined $10,000 for failure to adequately monitor and supervise residents identified with unsafe smoking practices. As a result, a resident suffered extensive burns to his face after smoking while receiving oxygen therapy. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Countryside Care Centre, a 207-bed skilled and intermediate care facility located at 2330 W. Galena Blvd., Aurora, has been fined $5,000 for failure to properly supervise a resident. The resident was taken to a nearby clinic, left unsupervised and was later found on the clinic’s lawn. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Jeffersonian Care Center, a 65-bed skilled care facility located at 1700 White St., Mount Vernon, has been fined $25,000 for sexual abuse of a resident by a staff member. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Sunny Acres Nursing Home, a 106-bed skilled care facility located at Rural Route 3, Petersburg, has been fined $10,000 for failure to assess a significant change in a resident’s condition. The resident had to be admitted to a local hospital with open sores. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Bloomington Rehabilitation and Health Care Center, a 78-bed skilled and intermediate care facility located at 1925 S. Main St., Bloomington, has been fined $10,000 for failure to supervise medication during distribution and to get a medical assessment after an error occurred. A resident took another resident’s medication and had to be treated at a local hospital. The facility has not yet requested a hearing on the Department’s action, but is still within the allotted time to do so.

Pekin Rehabilitation and Health Care, a 202-bed skilled care facility located at 2220 State St., Pekin, has been fined $10,000 for failure to provide Cardiopulmonary Resuscitation (CPR) in a timely manner to a resident found unresponsive. The facility has not yet requested a hearing on the Department’s action, but is still within the allotted time to do so.

Rosewood Care Center, Inc. of Edwardsville, a 120-bed skilled care facility located at 6277 Center Grove Road, Edwardsville, has been fined $50,000 for failure to adequately monitor and administer a resident’s pain medication. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Freeport Rehabilitation and Health Care Center, a 143-bed skilled care facility located at 900 S. Kiwanis Dr., Freeport, has been fined $10,000 for failure to assess and monitor the status of a resident, to provide the necessary higher level of care, to notify a physician of the changes in the resident’s condition and to provide ongoing assessment of the resident’s status. Due to a delay in treatment, the resident was admitted to the hospital. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Shelbyville Rehabilitation and Health Care Center, an 80-bed skilled and intermediate care facility located at 2116 S. 3rd and Dacey Dr., Shelbyville, has been fined $5,000 for failure to provide adequate supervision to a resident identified as a high-risk for wandering. The resident left the facility undetected, but was later found unharmed. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Pavillion of Forest Park, a 232-bed skilled care facility located at 8200 W. Roosevelt Road, Forest Park, has been fined $30,000 for failure to initiate CPR; to promptly obtain, act upon and notify a physician of a critical lab report; and to send a resident to the emergency department. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Colonial Apartments, a 16-bed intermediate care facility for the developmentally disabled located at 920 W. Fourth, Centralia, has been fined $10,000 for failure to develop a policy for residents while on a community outing and to supervise residents when returning from the activity. As a result, a resident was left at a community outing . The facility failed to report the incident to the Department. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Colonial Plaza, a 16-bed intermediate care facility for the developmentally disabled located at 618 W. Goodner, Nashville, has been fined $40,000 for failure to provide nursing services. The facility did not assess, monitor and follow up on residents’ illnesses, infections, falls and overall condition. The facility also sent residents to a hospital emergency department or physician’s office without accurate, complete and current information regarding physician orders, diagnosis, emergency contact information and complete information regarding signs and symptoms of illness. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Independence Place, Inc., a 16-bed intermediate care facility for the developmentally disabled located at 1705 S. Park Ave., Herrin, has been fined $10,000 for failure to assure medications were administered as physician-ordered and residents received their medications two hours late. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Glen Brook, a 16-bed intermediate care facility for the developmentally disabled located at Route 45 North, Vienna, has been fined $5,000 for neglect and mistreatment. A resident left the facility after being told he could not go to church until he bathed. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Illinois Veterans’ Home at Quincy, a 551-bed skilled care facility located at 1707 N. 12th St., Quincy, has been fined $50,000 for failure to monitor a confused resident who walked away from the facility unnoticed and was later found dead. The facility has requested a hearing on the Department’s actions. No hearing date has been set.

Wincrest Nursing Center Corporation, an 82-bed intermediate care facility located at 6326 N. Winthrop Ave., Chicago, has been fined $15,000 for failure to prevent one resident from harming another and placing other residents and staff at risk. A resident became verbally and physically abusive to other residents and staff after drinking. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Manorcare at Palos Heights, a 174-bed skilled care facility located at 7850 W. College Drive, Palos Heights, has been $10,000 for failure to provide CPR for 35 minutes to a resident found unresponsive, cold and presumed dead, and for not calling 911. The facility has not yet requested a hearing on the Department’s action, but is still within the allotted time to do so.

Friendship Home, a 49-bed skilled care facility located at 826 N. High St., Carlinville, has been fined $5,000 for failure to provide adequate supervision to a resident who left the facility without the staff’s knowledge. The resident was later found unharmed. The facility has not yet requested a hearing on the Department’s action, but is still within the allotted time to do so.

Westside Care Center, a 96-bed skilled care facility located at 601 N. Columbia St., West Frankfort, has been fined $20,000 for failure to prevent sexual abuse of a resident, and not reporting the incident to the proper authorities. The facility has requested a hearing on the Department’s actions. No hearing date has been set.
