Nursing Home Violations for the Month of July

SPRINGFIELD, Ill – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of July. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Asta Care Center of Bloomington, a 117-bed skilled facility located at 1509 North Calhoun Street, Bloomington, has been fined $22,500 for failure to protect residents from sexual abuse from another resident identified as a sexual offender. The facility also failed to assess the resident before admission. The facility has requested a hearing on the Department's action. A status in the case is April 3.

Avenue Care Center, a 155-bed skilled and intermediate care facility located at 4505 S. Drexel, Chicago, has been fined $10,000 for failure to ensure residents' environment remained free of environmental hazards after renovations violated rules for asbestos abatements. The facility has requested a hearing on the Department's action. A status in the case is April 28.

Davis House, a 15-bed intermediate care facility for the developmentally disabled located 4237 S. Indiana Avenue, Chicago, has been fined $20,000 for failure to provide adequate supervision and prevent neglect when a resident, who was inappropriately taking on staff responsibilities, placed a hot compress on another resident's leg causing thermal burns. The facility did not request a hearing on the Department's action.

Gowin Parc of Mattoon, a 24-bed sheltered care facility located at 300 Lerna Road South, Mattoon, has been fined for failure to adequately supervise a resident who went missing. The resident was found outside in 94 degree (Fahrenheit) weather unable to get back into the facility without assistance. The facility requested a hearing on the Department's action. A hearing was held and the facility paid $2,750.

Helia Healthcare of Energy, a 159-bed skilled, intermediate and intermediate care facility for the developmentally disabled located at 210 East College, Energy, has been fined $7,500 for failure to adequately supervise a resident identified at risk for elopement after previous attempts to leave the facility. The resident left the facility and was later found lying in the street with cuts to both knees from falling.

The facility did not request a hearing on the Department's action.

Lakeview Living Center, a 145-bed intermediate care facility for the developmentally disabled located at 7270 S. Shore Drive, Chicago, has been fined $23,500 for failure to ensure immediate protective health measures were taken for 22 residents exposed to Hepatitis A while attending a workshop. The residents did not receive Immune Globulin after the facility was notified of the exposure. A resident was later diagnosed with Hepatitis A. The facility requested a hearing on the Department's action. A status conference was held March 10 and settlement negotiations are underway.

Manor Court of Peoria, 50-bed skilled care facility located at 6900 N. Stalworth Drive, Peoria, has been fined $40,000 for failure to develop and implement a system to identify patterns and causes of falls. The facility also failed to monitor residents to prevent recurring falls which led to serious injury for three residents identified at risk for falls. The facility has requested a hearing on the Department's action.

Manorcare at Palos Heights, a 130-bed skilled care facility located at 11860 Southwest Highway, Chicago, has been fined $20,000 for failure to ensure the pharmacist reviewed all medications prior to dispensing and to ensure the dose was appropriate for the resident's age and condition. After receiving the wrong dosage for 10 days, the resident suffered a cardiopulmonary arrest and died. The facility requested a hearing on the Department's action. A status conference in the case was held February 26.

Manorcare at Rolling Meadows, a 155-bed skilled care facility located at 4225 Kirchoff Road, Rolling Meadows, has been fined $45,000 failure to provide necessary care and services to avoid physical harm to a resident. The facility also failed to follow up on a physician order and to notify the physician in a timely manner regarding a resident's condition. These failures led to a resident's condition worsening and requiring a hospital stay. The facility has requested a hearing on the Department's action. A status conference in the case is April 3.

Mason Point, a 194-bed skilled, intermediate and sheltered care facility located at One Masonic Way, Sullivan, has been fined $50,000 for failure to maintain visual control for a resident when a body alarm was not in use. This failure resulted in the resident falling and suffering extensive head injuries requiring hospitalization. The resident was released from the hospital and later died. The facility has requested a hearing on the Department's action. A status conference in the case is June 4.

North Logan Healthcare Center, a 108-bed skilled care facility located at 801 N. Logan Avenue, Danville, has been fined $10,000 for failure to implement emergency procedures and failure to honor an advance directive request by a resident to be resuscitated. Staff did not conduct CPR on the resident after finding the resident unresponsive. The facility did not request a hearing on the Department's action.

Renaissance Care Center, a 143-bed skilled care facility for residents under the age of 22, located at 1675 E. Ash Street, Canton, has been fined for failure to follow its plan and procedures to prevent and investigate a resident who eloped. The resident left the facility without staff knowledge and was found by a passerby approximately one-quarter mile from the facility. The facility requested a hearing on the Department's action. A hearing was held and the facility paid $1,908.

Sullivan Rehab & Health Care Center, a 123-bed skilled care facility located at 11 Hawthorne Lane, Sullivan, has been fined $20,000 for failure to maintain a multi-position chair in safe functioning condition for a resident. This failure resulted in the resident falling and sustaining life threatening injuries which resulted in death. The facility requested a hearing on the Department's action. A status conference in the case is April 7.

Westchester Health & Rehab Center, a 120-bed skilled care facility located at 2901 S. Wolf Road, Westchester, has been fined $20,000 for failure to provide necessary care and services to a resident who was inadequately monitored and treated. These failures resulted in the resident being hospitalized for sepsis. The facility requested a hearing on the Department's action. A status conference in the case is April 2.
