Nursing Home Violations for the Month Of February

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of February. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Biltmore Rehab & Nursing Center, an 80-bed skilled and intermediate care facility located at 1701 W. 5th Avenue, Belvidere, has been fined $25,000 for failure to supervise a resident who repeatedly exhibited inappropriate sexual behavior toward female residents. Staff documented occasions of the resident grabbing, touching and kissing, and resisting attempts to redirect his behavior. The facility has not requested a hearing on the Department’s action.

Fountainview, a 135-bed skilled and intermediate care facility located at 1001 A. Jefferson Street, Eldorado, has been fined $20,000 for failure to provide medical services and monitor a resident’s specific condition for 25 days. The facility also failed to obtain and report the results of a test that had been ordered by the resident’s physician. The resident was admitted to the hospital for seven days of treatment. The facility has not requested a hearing on the Department’s action.

Harrisburg Care Center, a 68-bed skilled care facility located at 1000 W. Sloan, Harrisburg, has been fined $27,500 for failure to protect a resident from abuse by staff. A resident who was restrained by two staff suffered bruises and skin tears. The facility also failed to thoroughly investigate and did not follow its established policy regarding timely reporting and investigating of allegations of abuse. The facility has not requested a hearing on the Department’s action. 

Illinois Veteran’s Home at Manteno, a 340-bed skilled care facility located at One Veteran’s Drive, Manteno, has been fined $20,000 for failure to adequately monitor and supervise a resident according to a care plan. The resident choked on food while eating and died as a result. The facility requested a hearing on the Department’s action. A hearing in the matter is set for October 29.

Meadow Manor, a 96-bed skilled and intermediate care facility located at 800 McAdam Drive, Taylorville, has been fined $5,000 for failure to provide safe hot water temperatures at all locations in the building. At some areas of the facility, hot water temperatures measured between 139 to 143 degrees Fahrenheit (F), but should measure 100 to 110 degrees. No resident were reported injured due to excessive hot water temps. The facility has requested a hearing on the Department’s action. A status in the matter is set for September 10.

Odd Fellow-Rebekah Home, a 162-bed skilled care facility located at 201 Lafayette Avenue East, Mattoon, has been fined $30,000 for failure to protect residents from involuntary seclusion, physical and mental abuse, and not immediately reporting and investigating allegations of abuse. A nurse physically held down a resident forcing the resident to take medication and threatened another resident with an injection if that resident did not take medications. The facility requested a hearing on the Department’s action.

Rest Haven W. Christian Nursing Center, a 241-bed skilled and sheltered facility located at 3450 Saratoga Ave., Downers Grove, has been fined $25,000 for failure to supervise a resident who was identified on swallowing precaution. The resident, who required assistance eating, choked and died. The facility has requested a hearing on the Department’s action. A status in the matter is set for September 26.

South Shore Nursing & Rehab Center, a 240-bed skilled care facility located at 2649 East 75th St., Chicago, has been fined $25,000 for failure to supervise a resident who wanders in and out of residents’ rooms. This failure resulted in the resident entering another resident’s room and sexually assaulting that resident. The facility has requested a hearing on the Department’s action. A status in the matter is set for September 27.
