Nursing Home Violations for the Month Of May

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of May. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Adloff Place, a 16-bed intermediate care facility for the developmentally disabled located at 50 Adloff Land, Springfield, has been fined for repeat Type A violation for failure to comply with its plan of correction from an August 2006 survey. The Department sent the facility notification of license revocation. The facility requested a hearing on the Department’s action. A hearing was held. The final order was signed and the facility paid a fine of $23,000.

Alden of Waterford, a 99-bed skilled care facility located at 2021 Randi Drive, Aurora, has been fined $40,000 for failure to supervise and monitor residents to prevent falls, analyze reasons for falls and implement interventions to prevent falls. These failures resulted in a resident suffering injury after falling. The facility requested a hearing on the Department’s action.

All Faith Pavilion (formerly William L. Dawson), a 245-bed skilled care facility located 3500 S. Giles Ave., Chicago, has been fined $25,000 for failure to ensure that residents receive the necessary care and services to maintain adequate nutritional intake and to decease the risk of aspiration. As a result a resident was admitted to the hospital with aspiration pneumonia and sepsis and died. The facility has requested a hearing on the Department’s action.

Continental Care Center, a 208-bed skilled care facility located at 5336 N. Western Avenue, Chicago, has been fined $5,000 for failure to supervise a resident with a documented history of smoking non compliance. A fire broke out in the resident’s room and careless smoking is the suspected cause. The facility has requested a hearing on the Department’s action. A status in the matter is set for December 10.

The Elms, a 98-bed skilled care facility located at 1212 Madelyn Ave., Macomb, has been fined $40,000 for failure to evaluate the circumstances of falls, failed to access residents for causes of falls, failed to review and revise the care plan interventions to address the falls, and failed to implement interventions to minimize risk for falls and injuries. The facility requested a hearing on the Department’s action. A hearing is set for February 12.

Embassy Health Care Center, a 171-bed intermediate and skilled care facility located at 555 W. Kahler, Wilmington, has been fined $20,000 for failure to (1) ensure that residents who are on medications that require monitoring for safe and therapeutic blood levels have those blood levels tested, (2) monitor signs and symptoms for overdose, (3) administer the appropriate dose of an anticoagulant. The facility has requested a hearing on the Department’s action. A status in the matter is set for December 17.

Maryville Manor, 120-bed skilled care facilities located at 2133 Vadalabene Drive, Maryville, has been fined $20,000 for failure to identify and evaluate risk factors, prevent avoidable pressure sores, implement interventions and modify interventions, as appropriate. Residents developed new pressure sores or existing pressure sores worsened. The facility has requested a hearing on the Department’s action. A status in the matter is set for December 10.

Mulberry Manor, an 80-bed intermediate care facility for the developmentally disabled located at 612 E. Davie Street, Box 88, Anna, has been fined $22,500 for failure to implement their policy to prevent neglect after failing to investigate, develop and implement preventive measures for a resident who had fallen 12 times and another resident who choked on food. The facility has requested a hearing on the Department’s action.

Pekin Manor, a 120-bed skilled care facility located at 1520 El Camino Drive, Pekin, has been fined $30,000 for failure to have a system in place to closely monitor residents with known aggressive behaviors. A resident hit another resident causing injury to an eye. The facility has not requested a hearing on the Department’s action.

Pinnacle Health Care of Berwyn, a 145-bed skilled care facility located at 3601 S. Harlem Ave., Berwyn, has been fined $15,000 for failure to initiate an immediate response to a person in cardiopulmonary arrest. This resulted in a resident not receiving the proper CPR management during cardiac arrest and dying. The facility has not requested a hearing on the Department’s action.

Pleasant Meadows Christian Village, a 109-bed skilled care facility located at 400 W. Washington, Chrisman, has been fined $52,500 for failure to provide care services for 3 residents to prevent and promote healing of pressure ulcers. Specifically the facility neglected to identify residents at risk for pressure relieving measures prior to skin breakdown. The facility has requested a hearing on the Department’s action. A status in the matter is set for November 29.

Riviera Manor, a 200-bed intermediate and skilled care facility located at 490 W. 16th Place, Chicago Heights, has been fined $23,500 for failure to supervise and prevent a resident from wandering from the facility unsupervised and inappropriately dressed for inclement weather. The facility has requested a hearing on the Department’s action. A status in the matter is set for December 10.

Warren Barr Pavilion, a 271-bed skilled care facility located at 66 W. Oak Street, Chicago, has been fined $5,000 for failure to adequately supervise a resident previously identified at high risk for elopement. The facility failed to monitor the doors and elevators to prevent the resident from leaving. Police later found the resident walking in 27 degree weather with no coat. The facility has not requested a hearing on the Department’s action.
