NURSING HOME VIOLATIONS FOR THE MONTH OF MARCH

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of March. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

Adloff Place, a 16-bed intermediate care facility for the developmentally disabled located at 50 Adloff Lane, Springfield, has been fined $10,000 for failure to develop and implement written policies and procedures regarding the immediate notification of abuse allegations. A staffer at the facility is alleged to have hit a resident. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Alden Park Strathmoor, a 189-bed skilled care facility located at 5668 Strathmoor Dr., Rockford, has been fined $5,000 for failure to assure the safety of a resident who was able to leave the facility through a window. The resident was found unharmed by police. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Bethany Health Care & Rehab Center, a 90-bed skilled care facility located at Resource Parkway, DeKalb, has been fined $5,000 for failure to assure the safety of a resident, who left the facility without staff knowledge. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Center Home for Hispanic Elderly, a 156-bed skilled and intermediate care facility located at 1401 N. California, Chicago, has been fined $35,000 for failure to protect its residents from harm. The facility failed to adequately access its residents’ needs for side rails or other restraints, which resulted in injuries. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Clark Manor Convalescent Center, a 273-bed skilled care facility located at 7433 N. Clark St., Chicago, has been fined $5,000 for failure to provide adequate supervision to a resident. The resident was able to leave a locked unit and walk out of the facility. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Danville Care Center, a 200-bed skilled and intermediate care facility located at 1701 N. Bowman, Danville, has been fined $40,000 for failure to follow its plan of correction from a May 24, 2005 survey by failing to perform 15-minute checks per care plan and failing to orientate and train agency staff to conduct adequate supervision of residents. A resident left the facility without staff knowledge and was later returned by local police. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Fair Acres Nursing Home, a 74-bed skilled and intermediate care facility at 514 E. Jackson, Du Quoin, has been fined $5,000 for failure to provide adequate supervision to a resident. The resident was able to walk out of the facility without staff knowledge. The facility has not requested a hearing.

Helia Healthcare of Energy, a 159-bed skilled, intermediate and intermediate care facility for the developmentally disabled at 210 E. College, Energy, has been fined $20,000 for using restraints for staff convenience, resulting in possible mental abuse. A resident was locked in a linen closet and two residents were tied to their beds with sheets. The facility also failed to promptly report and properly investigate an allegation of abuse. The facility has not requested a hearing.

Kenwood Healthcare Center, a 318-bed skilled and intermediate care facility located at 6125 S. Kenwood, Chicago, has been fined $25,000 for failure to provide a resident with needed care and sufficient hydration for several days. The facility also failed to follow doctor’s orders or notify medical care in a timely manner. The resident was admitted to a hospital and later died. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Manorcare of Palos Heights, a 174-bed skilled care facility at 7850 W. College Drive, Palos Heights, has been fined $25,000 for failure to provide insulin to a resident over a two-month period. The resident experienced high blood sugar levels and had to be hospitalized. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Mar Ka Nursing Home, a 76-bed skilled care facility located at 201 S. 10th St., Mascoutah, has been fined $15,000 and notified of possible license revocation for failure to follow its plan of correction and provide adequate supervision to prevent a resident from leaving the facility without staff knowledge. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Meadows, a 99-bed intermediate care facility for the developmentally disabled located at 3250 S. Plum Grove Road, Rolling Meadows, has been fined $10,000 for failure to implement its policy and procedure by not completing an incident report for alleged sexual abuse, for failing to notify the Department and resident’s doctor about the alleged abuse, and for failing to ensure a resident’s health and safety needs were met after surgery. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Nokomis Golden Manor, a 92-bed skilled care facility located at 505 Stevens St., Nokomis, has been fined $20,000 for failure to assess and monitor a resident for a change of condition, have an interim care plan to ensure that resident’s needs were being met, confirm a doctor’s order and obtain hospital reports. As a result the resident had not been given food or fluids for three days. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Palm Terrace of Mattoon, a 178-bed skilled care facility located at 1000 Palm Ave., Mattoon, has been fined $10,000 for failure to implement a monitoring system and provide supervision for an unsecured courtyard through which two residents left the facility unnoticed. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Piasa Manor, a 16-bed intermediate care facility for the developmentally disabled located at 110 N. Alby Court, Godfrey, has been fined $10,000 for failure to provide adequate supervision. A resident was able to walk out of the facility without staff knowledge. The facility also failed to report and investigate the incident. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Pinnacle Health Care, LLC, a 271-bed skilled and intermediate care facility located at 2222 W. 14th St., Waukegan, has been fined $20,000 for failure to assess and monitor the doses and times of medicines administered to a resident. As a result, the resident had to be hospitalized for high toxicity levels that required dialysis treatment. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Prairie Manor Nursing & Rehab Center, a 146-bed skilled care facility located at 345 Dixie Highway, Chicago Heights, has been fined $20,000 for failure to adequately supervise a resident known to need assistance during meals. As a result, the resident sustained serious burns. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Rest Haven W. Christian N. Center, a 241-bed skilled and shelter care facility located at 3450 Saratoga Ave., Downers Grove, has been fined $20,000 for failure to provide adequate supervision and assure the safety of a resident known for trying to leave the facility. The resident, who wheeled herself out of the facility and was later found flipped out of her wheelchair, sustained back and finger fractures. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Shabbona Healthcare Center, a 91-bed skilled care facility located at 409 W. Comanche Ave., Shabbona, has been fined $10,000 for failure to ensure that a resident's side rails were free of accident hazards. The resident was found entrapped between the side rail and the mattress. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Shawnee Christian Nursing Center, a 159-bed skilled care facility located at 1901 13th St., Herrin, has been fined $5,000 for failure to protect a resident from sexual abuse. A facility staff member who witnessed the alleged inappropriate sexual behavior directed toward a resident and did not tell the facility for three days. The facility has requested a hearing on the Department’s action. Status conferences are ongoing. No hearing date has been set.

Springwood Nursing & Rehab, LLC, a 97-bed skilled and intermediate care facility located at 1920 N. Main St., Rockford, has been fined $15,000 for failure to assist a resident with keeping a doctors appointment to refill pain medication prescription. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Torrence Place, a 16-bed intermediate care facility for the developmentally disabled located at 2601 223rd St., Sauk Village, has been fined $20,000 for failure to protect a resident, who sustained burns to 30 percent of body during a bath. Staff failed to report their observations that water was hotter than usual after maintenance was done weeks before the resident was burned. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Vermilion Manor Nursing Home, a 233-bed skilled and intermediate care facility located at 14792 Catlin-Tilton Road, Danville, has been fined $75,000 for failure to develop a prompt and thorough assessment of a resident, to identify changes in the resident’s condition and to report those changes to the physician. As a result, a resident was taken to the hospital emergency department, where the resident died. The facility has requested a hearing on the Department’s action. No hearing date has been set.
