NURSING HOME VIOLATIONS FOR THE MONTH OF APRIL

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of April. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

The ARC of Jacksonville, Ltd., a 93-bed intermediate care facility located at 1320 Tendick, Jacksonville, has been fined $50,000 for failure to assess and monitor a resident who refused to eat, drink or take prescribed medications, or notify the resident's doctors about deterioration of mental and physical health. As a result, the resident was transferred to the hospital and later died from dehydration according to an autopsy. The facility has requested a hearing on the Department’s action. No hearing has been scheduled yet.

Belhaven Nursing Home, a 221-bed skilled and intermediate care facility located at 11401 S. Oakley, Chicago, has been fined $5,000 for failure to provide adequate supervision to a resident with Alzheimer's disease, who was identified as at risk for leaving the facility. The resident left the facility without staff knowledge and was re-admitted the next day with multiple abrasions and bumps. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Clinton Manor Living Center, an 83-bed skilled and intermediate care facility for the developmentally disabled located at 111 E. Illinois St., New Baden, has been fined $5,000 for failure to implement their policies, to properly alarm or supervise exit doors and to provide adequate supervision to a resident with a diagnosis of profound mental retardation and a history of self-injurious behaviors. The resident was able to walk out of the facility without staff knowledge. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Columbus Manor Residential Care Home, a 189-bed intermediate care facility located at 5107-21 W. Jackson Blvd., Chicago, has been fined $20,000 for failure to track a change in behavior for a resident diagnosed with mental illness whose psychotropic medications had recently been reduced. The resident left the facility undetected and was later found by police and admitted to the hospital with hypothermia, hyponatremia, hypokalemia and depression. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Heather Health Care Center, a 173-bed skilled care facility located at 15600 S. Honore St., Harvey, has been fined $30,000 for failure to protect a resident from mental and sexual abuse. A resident was coerced by a staff member to have sex with another resident and promised cigarettes, if they cooperated with providing sexual favors. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Heritage Manor South-Beardstown, a 79-bed skilled care facility located at 8306 St. Luke's Drive, Beardstown, has been fined $20,000 for failure to immediately notify a doctor when a resident with diabetes experienced low blood sugar levels and for failure to give complete information to the doctor when he was notified. The facility also failed to respond to and treat signs and symptoms of hypoglycemia and failed to have policies and procedures established for staff to follow regarding hypoglycemia for residents with insulin dependent diabetes. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Hickory Nursing Pavilion, a 74-bed skilled, intermediate care facility located at 9246 S. Roberts Road, Hickory Hills, has been fined $15,000 for failure to provide adequate supervision to a confused resident identified as at risk for leaving the facility. The resident left the facility without staff knowledge and was found walking in traffic by police. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Mattoon Health Care Center, a 148-bed skilled care facility located at 2121 S. Ninth, Mattoon, has been fined $5,000 for failure to provide adequate supervision to a resident with memory problems. The resident left the grounds of the facility without the staff’s knowledge. The facility has not requested a hearing.

Momence Meadows Nursing Center, a 140-bed skilled care facility located at 500 S. Walnut, Momence, has been fined $20,000 for failure to protect its residents from harm. The facility failed to take steps to protect residents while being transported on a bus from accidental injury. A resident suffered a fractured thighbone due to an unexpected seatbelt release. The facility has requested a hearing on the Department’s action. No hearing date has been set.
