NURSING HOME VIOLATIONS FOR THE MONTH OF FEBRUARY

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of February. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

The statement of violation for each facility can be obtained by visiting the Department’s Web site at www.idph.state.il.us.

Homestead House, a 16-bed intermediate care facility for the developmentally disabled located at 905 N. Jefferson, West Frankfort , has been fined $15,000 for failure to protect its residents from harm. The facility failed to make sure water temperatures did not exceed 110 degrees Fahrenheit from all faucets and to safely monitor residents who used the faucets. As a result, a resident received second-degree burns to her foot while bathing. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Meadows, a 99-bed intermediate care facility for the developmentally disabled located at 2350 S. Plum Grove Road, Rolling Meadows, has been fined $10,000 for failure to follow the facility’s policy and procedures on abuse and to ensure a resident’s health and safety needs were met after surgery. The facility did not complete an incident report for alleged sexual abuse, notify the Department, conduct an investigation or notify the resident’s physician of the alleged sexual abuse. The facility has requested a hearing on the Department’s action, but a date has not yet been set.

Clark Manor Convalescent Center, a 273-bed skilled care facility located at 7433 N. Clark St., Chicago, has been fined $5,000 for failure to provide adequate supervision to a resident. The resident was able to leave a “locked” unit and walk out of the facility. The facility has requested a hearing on the Department’s action. No hearing date has been set.

Nokomis Golden Manor, a 92-bed skilled care facility located at 505 Stevens St., Nokomis, has been fined $20,000 for failure to asses and monitor a resident for a change of condition, have an interim care plan to ensure that resident’s needs were being met, confirm a doctor’s order and obtain hospital reports. As a result the resident had not been given food or fluids for 3 days. The facility has requested a hearing on the Department’s action and is scheduled for April 17, 2006.

Center Home for Hispanic Elderly, a 156-bed skilled and intermediate care facility located at 1401 N. California, Chicago, has been fined $35,000 for failure to assess, identify and revise a care plan concerning bed side rails and other restraints. As a result, six residents fell from their beds to the floor. The fine also is for a second violation in which the facility failed to get rid of a water cooler that dispensed hot water at 176.9 degrees Fahrenheit and burned a resident. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Manorcare at Palos Heights, a 174-bed skilled care facility located at 7850 W. College Drive, Palos Heights, has been fined $25,000 for failure to provide insulin treatment and services to avoid physical harm to a resident for two months. As a result, a resident experienced high blood sugars and was admitted to the hospital. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Fair Acres Nursing Home, a 74-bed skilled and intermediate care facility located at 514 E. Jackson, Du Quoin, has been fined $5,000 for failure to supervise a resident who left the facility unnoticed. This facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Helia Healthcare of Energy, a 159-bed skilled, intermediate care and intermediate care facility for the developmentally disabled located at 210 E. College, Energy, has been fined $20,000 for using restraints for staff convenience, resulting in possible mental abuse. A resident was locked in a linen closet and two residents were tied to their beds with sheets. This facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Pinnacle Health Care, L.L.C., a 271-bed skilled and intermediate care facility located at 2222 W. 14 th St., Waukegan, has been fined $20,000 for improper administration of medication, which led to a resident being overmedicated and required hospital care. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Shawnee Christian Nursing Center, a 159-bed skilled care facility located at 1901 13 th St., Herrin, has been fined $5,000 for failure to prevent the sexual abuse of a resident. The facility requested a hearing, which was held on April 13, 2006.

Alden Park Strathmoor, a 189-bed skilled care facility located at 5668 Strathmoor Drive, Rockford, has been fined $5,000 for failure to assure the safety of a resident who left the facility through a window. The facility has requested a hearing on the Department’s action, but a date has not yet been set.

Springwood Nursing & Rehabilitation, LLC, a 97-bed skilled and intermediate care facility located at 1920 N. Main St., Rockford, has been fined $15,000 for failure to provide services to help a resident keep a doctors appointment to refill pain medication. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Shabbona Healthcare Center, a 91-bed skilled care facility located at 409 W. Comanche Ave., Shabbona, has been fined $10,000 for failure to ensure that a resident’s side rails were free of accident hazards. As a result, a resident was found trapped between the side rail and mattress. The facility has requested a hearing on the Department’s action, but a date has not yet been set.

Prairie Manor Nursing and Rehabilitation Center, a 146-bed skilled care facility located at 345 Dixie Highway, Chicago Heights, has been fined $20,000 for failure to adequately supervise a resident during meals. The resident spilled coffee and received 4 th degree burns. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Palm Terrace of Mattoon, a 178-bed skilled care facility located at 1000 Palm Ave., Mattoon, has been fined $10,000 for failure to provide supervision for an unsecured courtyard and two residents left the facility unnoticed. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Kenwood Healthcare Center, a 318-bed skilled and intermediate care facility located at 6125 S. Kenwood, Chicago, has been fined $25,000 for failure to provide needed care to a resident who was admitted to the hospital and later died. The facility has requested a hearing on the Department’s action, but a date has not yet been set.

Rest Haven W Christian N Center, a 241-bed skilled and sheltered care facility located at 2450 Saratoga Ave., Downers Grove, has been fined $20,000 for failure to supervise the front door of the facility where a resident exited in a wheelchair. The resident was later found tipped over and had suffered back and finger fractures. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Adloff Place, a 16-bed intermediate care facility for the developmentally disabled located at 50 Adloff Lane, Springfield, has been fined $10,000 for failure to develop and implement written policies and procedures that prohibit abuse. This is after an initial allegation that staff abused a resident. The facility has requested a hearing on the Department’s action, but a hearing has not been set.

Piasa Manor, a 16-bed intermediate care facility for the developmentally disabled located at 110 N. Alby Court, Godfrey, has been fined $10,000 for failure to prevent a resident from leaving a facility, to put systems in place to prevent resident from leaving and to follow the facility policy for reporting missing persons. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Vermilion Manor Nursing Home, a 233-bed skilled and intermediate care facility located at 14792 Catlin-Tilton Road, Danville, has been fined $75,000 for failure to promptly and thoroughly assess a resident, to identify changes in the resident’s condition and to report those changes to the physician. As a result, a resident was taken to the emergency room. The resident died. The facility has requested a hearing on the Department’s action, but a date has not yet been set.

Bethany Health Care and Rehabilitation Center, a 90-bed skilled care facility located at Resource Parkway, DeKalb, has been fined $5,000 for failure to prevent a resident from leaving the facility. The facility has requested a hearing on the Department’s action, but a hearing date has not been set.

Torrence Place, a 16-bed intermediate care facility for the developmentally disabled located at 2601 223 rd St., Sauk Village, has been fined $20,000 for an incident in which a resident was burned over 30 percent of his body, because the bath water temperature was too high. The facility has not yet requested a hearing on the Department’s action but is still within the allotted time to do so.

Mar Ka Nursing Home, a 76-bed skilled care facility located at 201 S. 10 th St., Mascoutah, has been fined $15,000 and given notice of license revocation for failure to follow its plan of correction and to provide adequate supervision to prevent a resident from leaving the facility. The facility requested a hearing, which was held on April 13, 2006.
