NURSING HOME VIOLATIONS FOR THE MONTH OF JANUARY

SPRINGFIELD, Ill. – The Illinois Department of Public Health today announced the following violations of the Nursing Home Care Act for the month of January 2005.

The statement of violation for each facility can be obtained by visiting the Department’s Web site at www.idph.state.il.us and clicking on the phrase “statement of violation” contained in the paragraph about the facility.

Brighton Garden-Hoffman Estate , a 120-bed sheltered care facility located at 2150 W. Golf Road in Hoffman Estates, has been fined $10,000 (Statement of Violation) for failure to provide proper nursing care to a resident who died.

Coventry Village , a 130-bed skilled and sheltered care facility located at 612 W. St. Mary’s St. in Sterling, has been fined $5,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unsupervised.

Dearborn Court , a six-bed intermediate care facility for the developmentally disabled located at 520 S. Dearborn St. in Kankakee, has been fined $5,000 (Statement of Violation) for failure to provide nursing services in accordance with a resident’s needs when there was evidence of dehydration, decreased urinary output, constipation, low blood sugar, high blood pressure and numerous other health conditions.

Lincoln Square , a 15-bed intermediate care for the developmentally disabled located at 202 S. Main St. in Jonesboro, has been fined $10,000 (Statement of Violation) for failure to thoroughly investigate incidents of possible sexual abuse and to notify the Department of all incidents. The facility also failed to send the two residents to the emergency room for examination as per facility policy.

Marklund Children’s Home , a 30-bed skilled care facility for persons younger than 22 years of age located at 164 S. Prairie Ave. in Bloomingdale, has been fined $10,000 (Statement of Violation) for failure to ensure that an allegation of abuse and neglect was immediately reported to the administrator resulting in the alleged perpetrator of abuse being able to continue to work and have direct contact with residents.

Moultrie County Community Center , a 16-bed intermediate care facility for the developmentally disabled located at 240 E. State St. in Lovington, has been fined $20,000 (Statement of Violation) for failure to thoroughly investigate allegations of staff to client abuse and incidents of client to client abuse; to remove staff being investigated for abuse from contact with residents during the investigation; and to notify the guardians and the Department of allegations of abuse.

Norridge Healthcare & Rehab Centre, a 315-bed skilled and intermediate care facility located at 7001 W. Cullom Ave. in Norridge, has been fined $10,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unsupervised. The resident died from hypothermia and cold exposure.

Oak Park Healthcare Center, a204-bed skilled and intermediate care facility located at 625 N. Harlem Ave. in Oak Park, has been fined $10,000 (Statement of Violation) for failure to ensure that a resident was free from neglect, to provide timely treatment and to notify a physician of a wound condition that was worsening. The resident had both legs amputated after the facility failed to properly treat wounds that became infected with maggots.

River Park Healthcare Center , a 177-bed skilled care facility located at 2545 24 th St. in Rock Island, has been fined $5,000 (Statement of Violation) for failure to prevent a resident from leaving the facility unsupervised and to have a system in place to monitor exit doors.

Rolling Hills Manor , a 130-bed skilled care facility located at 3615 16 th St. in Zion, has been fined $10,000 (Statement of Violation) for failure to prevent two residents from leaving the facility unsupervised.

Rosewood Care Center of Peoria , a 120-bed skilled care facility located at 1500 W. Northmoor Road in Peoria, has been fined $10,000 (Statement of Violation) for failure to assess and analyze possible similarities in a series of falls from a low bed; to follow physician’s orders to remove side rails from a low bed; and to monitor and evaluate the effectiveness of an electronic monitoring device to effectively alert staff of falls. One resident, who fell from a low bed four times in two days, died after her head became lodged between the mattress and side rail.

Warren Park Nursing Pavilion , a 127-bed skilled and intermediate care facility located at 6700 N. Damen Ave. in Chicago, has been fined $10,000 (Statement of Violation) for failure to provide adequate supervision to a resident who was a non-compliant smoker. The resident and her roommate both sustained injuries requiring hospitalization after the resident set her mattress on fire.
