ROLLING MEADOWS FACILITY FINED
SPRINGFIELD, Ill. – Clearbrook Center has been fined $20,000 (Statement of Violation) by the Illinois Department of Public Health for violating the state Nursing Home Care Act. The 92-bed intermediate care facility for the developmentally disabled is located at 3201 W. Campbell St. in Rolling Meadows.
During an annual inspection, Department surveyors found:
- Five people left the facility unsupervised over a three-month period. A problem with the alarm system that alerts staff when a resident attempts to leave the facility was first noted during a Department inspection in September 2003, but was not corrected for nearly a year.
- A resident, who had a history of leaving the building, walked out the facility’s front door and was later escorted back by police after he was found at a park across the street. Despite a history of wandering from the facility, his whereabouts were not checked regularly.
- A female resident was in the bathroom when touched inappropriately by a male staffer. The administrator confirmed there was no clearly defined procedure about what is looked at during a body check, which could lead to undetected abuse.
- A resident abused two of his peers. In one instance, he punched his roommate, who suffered a bloody nose, and was observed striking another resident in the head. Despite these actions, the resident’s behavior program was not reviewed for possible revisions to prevent further attacks on other residents from occurring.
Clearbrook Center has requested a hearing on the Department’s action. No hearing date has been set.
